All clerkship activities will be under the direction of an experienced practitioner. The student will develop competency in the following critical elements of this model: communication with practitioners and patients, assessment and monitoring of drug therapy, disease state management, patient education and effective pharmacy management.     College: College of Pharmacy Department: Pharmacy Credit Hours: 4 The focus is on Rural Health Issues, Interdisciplinary Teams, Cultural Awareness and Community Empowerment. The student will be sensitized to issues regarding health care and medical practice in a rural setting and provided with an opportunity to apply communication skills and drug/disease management outside of urban hospital and clinic settings. They will learn through collaboration with other interdisciplinary team members, working with collaborative partners in the communities and people of different cultures. This rotation will expose the student to all facets of community pharmacy practice and focus on interpersonal skills, leadership skills, process and quality improvement, personnel management, business management, and staff education.     College: College of Pharmacy Department: Pharmacy Credit Hours: 4 Emphasizes delivery of pharmaceutical care in the long-term environment. Students will provide a widerange of clinical services through participation in medical team rounds, conducting drug regimen reviews and preparing drug information responses. Practice sites include longterm care facilities, community mental health foster care homes, substance abuse residential homes, hospice and home-care settings. Conferences will be scheduled to discuss patient cases and disease statement management. Pre- Requisites: PHPR 600. Typically Offered On Demand

    College: College of Pharmacy Department: Pharmacy Credit Hours: 4 This rotation is designed to provide students with patientcare experiences in the management of hospice patients, to provide an understanding of the rationale of hospice care, to familiarize the student with the unique pharmaceutical needs and patient specific dosage forms of the hospice patient and to assess efficacy of extemporaneous preparations, and to gain a hands-on understanding of palliative care.     College: College of Pharmacy Department: Pharmacy Credit Hours: 4 The homecare clerkship will provide the student an opportunity to develop the basic skills needed to provide pharmacy services in the homecare setting. These skills include communication, therapeutic monitoring, the principles of aseptic technique, the proper use of respiratory medications, and pharmacokinetic and nutritional assessment.     College: College of Pharmacy Department: Pharmacy Credit Hours: 4 Involves the student in the area of managed care under the directive of pharmacy services. Students will be exposed to a broad range of experiences such as formulary management, academic detailing, provider and member education, patient profile reviews, and new drug reviews.     College: College of Pharmacy Department: Pharmacy Credit Hours: 4 This one-month elective clerkship rotation is designed to provide students with exposure to a variety of acute and chronic pain management services and techniques available either in the inpatient or outpatient setting. Students will gain an understanding of the role of the pharmacist as an integral member of pain management therapy. Pre-Requisites: Completion of all didactic coursework in PharmD program & 4th year Typically Offered Fall, Spring, Summer

    College: College of Pharmacy Department: Pharmacy Credit Hours: 4 Involves the student with total patient care through in-depth monitoring and surveillance in the setting of adult cardiology. Students will gain advanced training in order to more accurately assess patient responses to drug therapy, better formulate appropriate patient specific regimens and more accurately communicate information to other health care professionals. Conferences will be scheduled to discuss specific topics in cardiology and pharmacotherapy and patient care issues. Pre-Requisites: PHPR 600. Typically Offered On Demand
