This course incorporates multiple aspects of the Synergy Model of Patient Care as the nursing competencies are more aligned with patient characteristics. The multidimensional problems associated with chronic conditions provide a context for the development of collaboration and systems thinking and the expanding role of the nurse. Principles of rehabilitation and demands for long term care are explored. Models of case management and the role of the case manager is examined within the framework of the health care delivery sytems, quality, cost/finances, legal and ethical issues.
