Chapron The student will apply knowledge of pharmacotherapy of acute and chronic thrombotic disorders to the provision of pharmaceutical care to patients requiring anticoagulation therapy. Emphasis is on the optimization of medication-related outcome in anticoagulated patients through past and current medication assessment, multidisciplinary treatment planning, efficacy and safety monitoring, and patient education.
