Discharge planning assists patients and their families in arranging appropriate health care services. The course will review the historical development and current demand for discharge planners, particularly in New York State, as well as techniques and resources in the planning of ambulatory, acute and long-term care, and new alternative health services for the individual patient. The methodology for the course will include guest speakers, on-site visits to institutions, and case studies. This course is intended for social workers, health counselors, nurses, patient care representatives, quality assurance and utilization review coordinators, and the general public interested in the options available for health care.
